Jaycen Don Ezekiel Smith, 9 days old, passed away Friday, January 27, 2017 at Franciscan Health Lafayette East in Lafayette.
Born on January 18, 2017 in Lafayette, he was the son of Jerald Smith and Tiffany Clemons of Logansport.
Other survivors include several brothers; one sister; paternal grandmother, Sandi Conklin-Smith; maternal grandmother, Barbara Latta; maternal great grandfather Mitchell Latta Sr.
A funeral service will be held at 11:00 AM on Thursday, February 2, 2017 in the Mt. Hope Cemetery Chapel with Pastor Todd Masters. Burial will follow in the Baby Walk section of Mt. Hope Cemetery.
